Application Process for New Substitute
Teachers and Support Staff

Applications should be submitted to the District Office,
1903 Bartlett Avenue during regular office hours, Monday
through Friday, 7:30 - 3:30.

Driving Directions

 

Teacher Subs - Please bring:

• Completed application (see certified application)
• Copy of teaching license (Wisconsin teacher license required)
• Completed Background check form
• Completed I-9 Form (complete Section 1)
• Completed W-4 Form (bottom of page 1)
• Completed Race/Ethnicity Questionnaire
• Completed Direct Deposit Authorization Form
• Social security card and drivers license, or passport

Certified teachers only.
 

Support Staff Subs - Please bring:

• Completed application (see support application)
• Completed Background check form
• Completed I-9 Form (complete Section 1)
• Completed W-4 Form (bottom of page 1)
• Completed Race/Ethnicity Questionnaire
• Completed Direct Deposit Authorization Form
• Social security card and drivers license, or passport
