Child Development Check-In
This is an opportunity for ongoing child development check-in, for children ages 3 and up until they enter kindergarten. As a family, it is important to know:

Typical child development (CDC Milestones Moments Online or App for a developmental chart of typical development.)

Where is my child on their developmental journey? (We will be providing a questionnaire for families to fill out and return.)

What resources are available in our community to support child development? (After the questionnaire is filled out, we will send results with activities and resources to support your child in their next steps of learning.)
At anytime, a family can request an Ages and Stages Questionnaire (ASQ3) to determine where your child is on their developmental journey. Once a year, we will formally send out a questionnaire to families with children between 3 and 4 years of age. We will be mailing these out in September. If you have not received a questionnaire or have concerns prior, please complete this form.
